Student W-9S Request

As a student of an accredited institution, you are entitled to receive a 1098-T Tuition Statement from Maine Media College.

In order to prepare this form for you to use when filing your taxes, we will need to have your Social Security Number on file.

Use of Social Security numbers: Maine Media College is required by federal law to report tuition along with Social Security numbers (SSNs) for all to whom tuition is paid. SSNs are maintained and used by Maine Media College for reporting to federal and state agencies in formats required by law. Maine Media College will not disclose a SSN without prior consent to anyone outside Maine Media except as permitted or required by law.

  • Check the Electronic Consent Disclaimer if you would like your 1098-T form delivered to your email.
  • Sign and date form.
  • Select “Click To Sign”.
